    DPS utilizes 8 to 18 to organize and manage our athletic programming, from sign-ups to schedules. Parents/guardians can create an account using the links below to sign up students for athletics at their school.

    Elementary School Athletics: 2nd-6th Grades

Sports Physicals

  • Sports physicals are required by the IESA and IHSA for all middle school and high school athletes. Sports physicals are valid for 13 months and must be obtained with documentation provided to your student's school prior to their participation in athletics, including practices. Click here for the sports physical form to be completed by your student's physician and provided to your student's school.

    Note: 6th and 9th grade school physicals count as sports physicals; however, sports physicals DO NOT count as required 6th and 9th grade school physicals.
